Minor Participant Waivers (Under 18)
We maintain three versions to accommodate different enrollment situations:
Full Long-Form Minor Liability Waiver (Comprehensive, 2-page version)
Condensed Parent-Friendly Minor Waiver (Simplified 1-page version)
Trial / Introductory Class Minor Waiver (Quick-sign format for short-term participation)
Each document explains the risks of martial arts training and confirms voluntary participation and assumption of risk by the parent/guardian.
Adult Liability Waiver
All adult participants must complete the Adult Liability Waiver prior to training. This document outlines assumption of risk, release of liability, and participant responsibilities.
